Depending on the form, this may include: your name, email address, phone number, professional credentials and license information, state(s) of licensure or intended practice, specialty, practice name and details, and any other information you choose to provide.

We do not knowingly collect information from anyone under 18. This site is intended for licensed healthcare professionals and students in accredited programs.

How We Use Your Information

We use the information you submit to: respond to your inquiry or application; evaluate fit for our consulting, preceptorship, collaborating provider, and website design services; deliver services you engage us for; communicate with you about your engagement; and operate and improve our business. We do not sell your personal information, and we do not share it with third parties for their own marketing.

How Your Information Is Stored and Shared

Form submissions are transmitted securely and stored in our client management system. We use trusted service providers to operate our infrastructure — including database hosting, application hosting, and email — who process data on our behalf under their own security and privacy commitments. We share information with these providers only as needed to operate the site and deliver our services, or as required by law.

Data Retention

We retain the information you submit for as long as needed to evaluate your inquiry, deliver services, and meet legal and business record-keeping obligations. You may request deletion at any time (see below).

If you are a California resident, the California Consumer Privacy Act (CCPA) gives you the right to know what personal information we have collected about you, to request a copy of it, to request that we delete it, and to not be discriminated against for exercising these rights. Cookies and Site Data

This site uses minimal browser storage to make our multi-step forms work (for example, preserving your progress between steps). We do not use advertising cookies or third-party tracking for advertising.
